2. Two letters of recommendation on letterhead from people who know you professionally. You must use the " Recommendation Form" and follow the instructions on the form. One of the letters MUST be from a college science professor. The second recommendation can be from a community leader, teacher, coach, counselor, etc. (Recommendations can be uploaded as part of the online application or emailed to CDI@ohsu.edu )
3. Career goals essay
