School of Nursing scholarships are awarded using information provided on this application.

- Students will not be considered for OHSU nursing scholarship funds without this completed application form and essay.

- Students must be admitted and in good academic standing in order to be considered for a scholarship.

ONLINE APPLICATION GUIDELINES

ESSAY: Please prepare your scholarship essay before you begin filling out the this online application. You will be asked to copy and paste the entire essay into the one of the application's questions.

The purpose of the HealthE STEPS 2.0 scholarship is to improve health equity within communities through professional nursing practice. Describe your educational and professional goals in relation to the purpose of this scholarship. Please describe your leadership experience and community service. Please limit your essay to 750 words.

You will be given a validation code, which you will need to copy and save, as you will be required to enter the code to return to finish it.

Thank you for your interest in the advancing Health Equity through Student Empowerment & Professional Success (HealthE STEPS) scholarship program. The HealthE STEPS grant provides academic, social and financial support to pre-nursing and nursing students from underrepresented and disadvantaged backgrounds.

Eligibility:
Applicants must be a citizen of the United States, a non-citizen national of the United States, or a foreign national who possesses a visa permitting permanent residence in the United States.

Applicant must also be a nursing students or incoming nursing student at one of the following:
1. Nursing students in the OHSU 3 Year Baccalaureate program (all 5 campuses)
2. Nursing students in the Accelerated Baccalaureate Program, Class of 2020 (Ashland & Portland)
3. Nursing students in the on-campus transition Baccalaureate Program (CC transition)

To be eligible for scholarship awards or stipends applicants must also participate in the following activities:

a. Expected to maintain regular communication with diversity coordinator.
b. Expected to attend regularly scheduled meeting with diversity and/or faculty coordinator.
c. Submit grades to diversity coordinator each term.
d. Participate in grant-sponsored activities, including mentoring, outreach events, expanded orientation programs and workshops.
e. Access academic support services as recommended.
f. Remain in good academic standing.
g. Provide demographic information requested and participate in evaluation activities.

You may save and return to your work if you are not able to complete in one session. We encourage you to answer all questions to the best of your ability.

By submitting this application you authorize the OHSU School of Nursing to release the information you provide in this application to the HealthE STEPS 2.0 Selection Committee.
